For conspicuous gallantry and devotion to duty on June 12th 1917 when a raid was made on the enemy trenches west of LENS. Captain Collins was in command of the Company carrying out the raid. Soon after leaving our trenches he was hit on his steel helmet by shrapnel and was stunned for a minute or two. A few minutes later he was again wounded in the leg. He was incapacitated from climbing the slap-heap to the objective, but he refused to be taken back and waited till the Company withdrew. As he followed them back he came across a wounded N.C.O. and would not leave him until a stretcher had been fetched and the N.C.O. carried away. He (and the C.S.M. who was with him) were the last to return to our trenches, and remained in "no man's land" for a long period and until certain that the wounded had been got back. By his personal gallantry during the raid, especially after being wounded and by his unfailing cheerfulness and enthusiasm Captain Collins set a splendid example to his men. He has done much good work and was mentioned in the last Honours list. N.B. Strength of raiding party 2 officers and 102 other ranks.
