(Recommended for Military Medal)
For conspicuous gallantry and devotion to duty on July 1st 1917 when the Battalion attacked the enemy positions at CITE DU MOULIN, S.W. of LENS. The left of the attack was held but immediately to the right of this Sgt. Chapman pushed forward with his Platoon and cleared the houses in front of him. Finding that he had lost touch with his left, he at once dug in and sent out patrols to get in touch with the left. This he succeeded in doing. Throughout the day he kept in constant touch with the Companies on either flank and this established a connecting link between them. He lost no opportunity in generally harassing the enemy and opening fire whenever a target presented itself, and he personally accounted for several of the enemy. All this was done under very heavy artillery, machine-gun and rifle fire, and he displayed great gallantry and initiative throughout. He has always proved himself a gallant and reliable N.C.O.

(Recommended for Distinguished Conduct Medal)
For conspicuous gallantry and devotion to duty on several occasions, notably: 1. On June 19th 1917 when enemy trenches near FOSSE 3 DE LIEVIN, S.W. of LENS were attacked and captured. The Company, in which this N.C.O. commanded a Platoon, was checked by heavy machine-gun fire and suffered considerable casualties, the Company Commander being wounded. Sgt. Chapman showed excellent qualities of leadership and courage, and rendered most valuable assistance in organising a further attack, which was completely successful. 2. On July 1st 1917, when the enemy positions at CITE DU MOULIN S.W. of LENS, were attacked, this N.C.O. again displayed qualities of leadership and personal gallantry in pushing forward with his Platoon and clearing houses. This N.C.O. has served continuously with his unit for a period of thirty months in this country and has done consistent good work, being always cool and cheerful under trying conditions, and having the utmost confidence of his men. He sets a splendid example. At the attack on the HOHENZOLLERN REDOUBT on October 13th 1915 he did especially good work, but was not brought to notice owing to no officer of the Battalion being present, and the facts not becoming known until some considerable time later.
The above citation appears to have been summarised for the purposes of the London Gazette: For conspicuous gallantry and devotion to duty at all times. He invariably showed excellent qualities of leadership and the greatest courage in action, and rendered most valuable assistance in organising successful attacks. He was always cool and cheerful under trying conditions.
